ATHLETIC ELIGIBILITY CLEARANCE PROCEDURE




Participating in the Athletic Program at Samohi requires students to be academically eligible and complete the necessary paperwork to be cleared for every sport.  Please make sure that ALL forms have been read, filled out completely, signed and/or initialed by parent/guardian, student, and/or physician and returned to the Athletics Office.

Please read the following information:
  1. Pursuing Victory with Honor, Code of Conduct for Student Athlete, and Code of Conduct for Parent/Guardian.
  2. Athlete's Code of Ethics.
  3. Illegal use of controlled substances and performing enhancing drugs. (#5 on Parent Permision Form.)
  4. The law and policies regarding Hazing.
Athletic clearance procedure requires parent/guardian and student to return the following items signed and initialed where required:
  1. Sign and return: Parent Permission Form/Informed Consent - This form acknowledges that you (parent/guardian and student) have received, read, and understand the listed policies, rules, and possible injuries the athlete may suffer.
  2. Sign and return: Athletics Health/Emergency Card - Two sets must be completed and signed by parent/guardian.
  3. Sign and return: Medical Clearance Form - Parent/Guardian completes bottom of form and signs it. The top must be completed and signed by physician.
  4. Bring: Proof of Insurance - A copy of current health insurance card showing athlete as insured or proof that your athlete is insured under cardholder policy. If an athlete has no insurance, coverage may be purchased through Meyer-Stevens (Allianz) insurance prior to participating. (Insurance applications are available in the Athletics Office). Student must have insurance in order to participate.
  5. Bring: $95.00 to cover the following for the first season sport (second season sport additional $75, third season sport additional $75).
    $20.00 Sports Pass. (This includes admission to all regular season home and away football and basketball games; it does not include CIF Playoff Games). Non-athletes pay $25 for a Sports Pass.
    $25.00 Athletic Trainer Contribution. (The District and Samohi contribute approximately 60% of the cost).
    $25.00 Transportation Contribution. (The District pays 65% of the cost).
    $15.00 Officials and Equipment replacement.
    $10.00 Facility Maintenance and repair.

In order to be eligible the student must have maintained a minimum 2.0 grade point average of a 4.0 scale in all enrolled courses during the previous grading period (per the California Interscholastic Federation – Southern Section Bylaws, Article 1 Section 205 Scholastic Eligibility).

Once the student has filled out the packet for one team, they are able to use that same physical and paperwork for any other sport they plan on trying out for during any season within the same school year at Santa Monica High School.

Please make all checks payable to: Santa Monica High School and make sure to include the student's name and ID number is on the memo line.


Santa Monica High School offers a wide range of interscholastic and intramural athletics to meet the needs and interest of all students. Most sport activities offer three levels to accommodate all abilities:
  • Varsity for the Advanced
  • Junior Varsity for the Intermediate
  • Freshman/Sophomore for the Beginners

STUDENT PARTICIPATION:
An athlete may participate in only one sport per season. Athletes cannot change from one sport to another during the season except with approval of both coaches and the Athletic Director. When an athlete is removed from a sport for disciplinary reasons, he/she may not participate in any sport for the remainder of the semester and for the following semester.
