• Phone number
• Number of Transcripts requested
• YOUR SIGNATURE
• Date signed

For a copy of your Diploma, it takes at least one month after order has been received. The fee is $30.00.

You can also request a copy of your Health Record or Test Scores. The fee is $3.00 each.
