• Address where you want the transcript mailed

• Date of graduation or last date of attendance
• Name of organization and/or individual and mailing address (Where to send.)
• Fee: $3.00 per Transcript
• Checks or money orders made payable to: Santa Monica High School.
• Phone number
• Number of Transcripts requested
• YOUR SIGNATURE
• Date signed

For a copy of your Diploma, it takes at least four to six weeks after order has been received by the processing plant.  The fee is $30.00.

You can also request a copy of your Health Record or Test Scores. The fee is $3.00 each.
